Manage Policy Forms on Programs

Introduction

This article explains how to associate and configure Policy Forms with Programs in BindHQ. Policy Forms allow you to define the documents that will be included in quotes and policies for specific programs or products.

Note: Programs are available only to customers on the Professional tier and above.

To attach policy forms, you will first need to Create a Quote Template.

Add & Configure Policy Forms

Access Policy Forms

  1. Navigate to Settings.

  2. Select Customization.

  3. Under Dynamic Documents, choose:

    • Create New Document, or

    • Browse, View & Edit Existing Documents

To Create a New Policy Form

  1. Click Create New Document.

  2. Select Policy Form from the Document dropdown.

  3. Choose the Type:

    • Create Custom Document, or

    • Upload Fillable PDF

  4. Complete the setup and click Save.

Note: Once a document is created, the Document and Type cannot be changed.

 

Policy Form Usage

Each Policy Form must be assigned a Usage Type, which determines how it behaves within a Quote Template.

  • Mandatory: The form is automatically added to the quote and policy and cannot be removed by the underwriter.

  • Optional: The form is available to be added to the quote or policy but is not selected by default and must be manually included.

  • Default: The form is automatically selected for the quote and policy but can be removed by the underwriter if needed.

  • Custom: Selecting Custom enables the Manage Rules option, where you can define rule-based behavior for the form. Within Manage Rules, you can assign the form a usage type of Default, Mandatory, Optional, or Excluded based on specific criteria using logical conditions (AND / OR).

    • Excluded (within Custom rules): The form is not added to the Quote Template. It will not be visible and cannot be added to a quote or policy.
