Manage Policy Forms on Programs

Understand how to configure Policy Forms on Programs.
This article explains how to associate and configure policy forms with programs in BindHQ, allowing you to create custom programs or products. Please note that programs are only available for customers on the Professional tier and above.

Add & Configure Quote Templates
Admins can access the Quote Templates by navigating to Admin in the sidebar menu, then selecting Markets & Carriers and Add & Edit Quote Templates and Programs.
To modify an existing Quote Template or Program, open it directly. To create a new one, click New Template and then select Quote Template.
Add & Configure Policy Forms
To add Policy Forms to a Quote Template, go to Admin in the sidebar menu and select Templates & Documents. From here, you can either choose Add New Policy Form or click Browse, View & Edit Policy Forms.
To create a new template, click the blue Add New Template button. This button also allows you to add a new Quote Document, Email Attachment, or Email Template.
Policy Form Usage
The supported Policy Form usage types are as follows:
Excluded: The form is not added to the Quote Template, or custom rules are configured. The Policy Form will not be visible and cannot be added to the quote or policy.
Optional: The Policy Form is available to be added to the quote or policy, but it is not selected by default. The underwriter can choose to include the form during quoting or when creating an endorsement. Examples include additional coverage forms or exclusions.
Default: The Policy Form is available and selected by default for both the quote and policy. The underwriter can choose to remove it during quoting or when creating an endorsement. Examples include additional coverage or exclusion forms.
Mandatory: The Policy Form is automatically added to the quote and policy and cannot be removed. Examples include declaration pages, schedules of forms, etc.
Custom: The Policy Form can be configured with a default usage type, and it can also have a different usage type based on specific criteria. BindHQ supports logical expressions like "And" and "Or" for these rules. Examples include forms specific to a class code or state.
Policy Issuance: Common Declarations Statement, Notification of Claims, Services of Suit, Signature
The standard "Common Dec Page" document that comes with each Installation supports dynamic fields, which can be edited here. Each section will be printed on the "Common Dec Page" document. The text fields support template replacements, if dynamic data is needed. The signature dropdown shows the available signatures to be printed on the Common Declarations document. These are managed on Settings / Company Settings / Company Profile.
How Form Configuration Rules are Triggered on Quotes and Endorsements
When you update a Quote or Endorsement using a Program with Form Configuration Rules, the forms are dynamically updated in real-time. Based on your input, Policy Forms are adjusted to always reflect the current state of your Quote or Endorsement.
If you notice a small icon on the Forms tab, it indicates that a change in the risk data has triggered an update to the forms on the quote or policy.
For example, if the Form Configuration Rules in the Quote Template include a form titled "Theft Exclusion," which is triggered when the Cause of Loss is set to "Special Excluding Theft," the form will not initially be added if the Cause of Loss is "Special Including Theft." However, once you update the Cause of Loss to "Special Excluding Theft," the rules are activated, and the form is automatically added to the quote.
To confirm the changes, simply click Save. If you choose to cancel the edit, both the risk data and form configuration will remain unchanged.
Changes to a quote template will not affect quotes and policies, regardless of their status. To refresh the Form Configuration on quotes, create a new quote, that will contain the new rules.
